Initial Comments:
An on-site follow-up survey conducted on 08/13/2021, found that St. Luke's Tamaqua Dialysis, had corrected the following standard deficiency that was determined to be in substantial compliance with the following requirements of 42 CFR, Part 494, Subparts A, B, C, and D, Conditions for Coverage of Suppliers of End-Stage Renal Disease (ESRD) Services. The deficiency was cited as a result of a complaint investigation survey on April 30, 2021 and completed June 3, 2021.
